SWPA Area Agency on Aging

The mission of Southwestern Pennsylvania Area Agency on Aging, Inc. is to promote the well-being of older adults through a planned, coordinated and collaborative program of health and social services.

The Area Agency on Aging subcontracts with 34 local agencies to provide a comprehensive service delivery system for older adults. Wherever feasible, consumers are given the opportunity to choose among a variety of qualified providers. While many services are provided through the development of subcontracts, others are provided directly by the Area Agency on Aging.

Senior Community CentersThe Agency provides a full range of programs and services including:

  • adult day care
  • advocacy
  • employment assistance
  • home and community-based services
  • home-delivered meals
  • health promotion
  • information and assistance
  • legal assistance
  • person centered counseling
  • protective services
  • senior community centers
  • transportation

What is CHC?
COMMUNITY HEALTHCHOICES (CHC) is Pennsylvania’s mandatory managed care program for dually eligible individuals and individuals with physical disabilities

Who will enroll in CHC?
Individuals will be enrolled in CHC if they are 21 years old or over and are:
• Receiving both Medicare and Medicaid; OR
• Receiving LTSS in the Attendant Care, Independence, COMMCARE, or Aging waivers; OR
• Receiving services in the OBRA waiver AND determined nursing facility clinically eligible; OR
• Receiving care in a nursing home paid for by Medicaid.
• An Act 150 participant who is dually eligible for Medicare and Medicaid.

Will individuals have a choice of CHC MCOs?
YES. Each CHC participant may choose his or her MCO.
The MCOs are AmeriHealth Caritas, PA Health & Wellness, and UPMC Community HealthChoices.

Southwestern Pennsylvania Area Agency on Aging, Inc., is pleased to announce the following special activities to promote the health and wellness of older adults who reside in Fayette, Greene and Washington Counties, or who are members of senior centers within the counties.

Washington County Senior Expo 30th Anniversary ~ Thursday, September 20, 2018 ~ Washington Crown Center Mall, 1500 W. Chestnut Street, Washington, Pa. From 10 a.m. to 2 p.m. Flu shots will be available.

Greene County Walk to End Alzheimer’s ~ Saturday, September 22, 2018 – Wana-B Park, Ceylon Road, Carmichaels. Pa. Registration is at 9 a.m. Ceremony at 9:45 a.m. for the Walk at 10:15 a.m. The route is 2 miles.

Greene County Elder Justice Day ~ Thursday, September 27, 2018 ~ Valley View Farm, 108 Orndorff Road, Waynesburg, Pa. 9 a.m. to 1 p.m. Keynote Speaker will be Pennsylvania Department of Aging Secretary Teresa Osborne.

Greene County Senior Fair ~ Friday, October 5, 2018 ~ Greene County Fairgrounds,
Waynesburg, Pa. From 10 a.m. to 2 p.m. Flu shots will be available.

Washington County Walk to End Alzheimer’s ~ Saturday, October 6, 2018 ~ Wild Things Park, One Washington Federal Way, Washington, Pa. Registration is at 8:30 a.m. Ceremony at 9:30 a.m. for the Walk at 10 a.m. The route is 2 miles.
