Person Centered Counseling is a person-centered decision support process that helps individuals make appropriate choices about their needs, preferences, and values.
Our Person-Centered Counselors can help:
- Empower the individual to take action regarding their needs
- Support decision-making
- Develop a personalized Long-Term Services and Supports plan
- Make connections to other community resources and programs
Eligibility
Adults aged 60 and older and adults of any age with a disability who live in Fayette, Greene, or Washington counties are eligible.

Once we receive the form, staff will call the person needing help to gather more information and start the process.
